How do I let YI know if my child has a medical or physical limitation?

Your group coordinator will provide each participant with a registration form. Please provide important medical and physical information that should be considered in planning safe activities for your child. 

On the registration form you should state any medical or physical limitations, and highlight medications that need to be dispensed by an attending chaperone. Make a point of contacting your group coordinator by phone to make your concerns known.
